How to calculate the amount (Per person) –
For a stay up to 5 days the amount is fixed. For a stay between 6 to 10 days - multiply the
amount into the number of days.
For stay of more than 11 days - multiply the Daily Quota into the number of days and add the
Fixed quota amount.

Visa Applicants Are Hereby Requested To Take Note That Passports On Which The Stamp “Passport To Be Shown On Return From Italy” Is Affixed, Are Obliged To Show The Same To The Consulate General For Verification, Within 15 Days After Arrival In India.
Failing which the Police Headquarters in Italy will be informed for Necessary Action.
Copies of the Passport, Visa and Arrival/Departure stamps are to be Presented as per any of the following:

At the time of submission of documents at VFS, applicants need to attach old passport copies (first and last page of all previous passports) along with copies of old US, UK and Schengen visas.

AFFIDAVIT FROM BOTH PARENTS
An Affidavit is required for a minor travelling to Italy with parents / without parents / with a single parent and is a mandatory document. This affidavit has to be attested and notarized by Under Secretary of Home Department from your place of residence. The sample copy of the Affidavit is attached for reference. If the parents are divorced then also the affidavit is required from both parents. Also passport copies of both parents are to be submitted along with the affidavit mandatorily with signatures of both parents on the affidavit.

OR
CONSENT LETTER FROM BOTH PARENTS
Alternatively, a Consent Letter needs to be executed by both parents, with a prior appointment at the Italian Consulate, and both parents have to sign the same in the presence of a visa officer.
In such case, the personal presence of the minor is not required and all visa applications may be submitted by the parents. The said application forms have to be signed by both the parents.
This is also to inform that even if the minor/s travel with both the parents, the consent letter has to be executed by both parents.
